Children ADHD Evaluation
An evaluation for Attention Deficit/Hyperactivity disorder requires a team approach. The team consists of the patient, your physician, and one or more observer.
Our portion of the evaluation consists of gathering information about a patient's social, emotional, academic and health history. For children, a questionnaire is filled out by the parent(s), one or more teachers and sometimes the child, depending on the age.
Adult ADHD Evaluation
For adults, a questionnaire is filled out by the patient and an observer.
The information from the questionaire and the evaluation are used to determine whether or not the client falls in the diagnostic criteria from the Diagnostic Statical Manual for Mental Disorders (DSM-IV) for Attention Deficit/Hyperactivity Disorder.
This evaluation is spread out over 2 appointments. A full report is provided, with your permission, for your physician to read and determine the course of action.
